Disclosure Authorization Form
Authors of accepted oral and poster abstracts will be required to submit a Disclosure Authorization Form (or your agencies Approved for Public Release form)
for all documents associated with the conference (abstract, manuscript and oral/poster presentation).
The completed form(s) must be submitted by 20 October 2008 (updated) and can be emailed to 2008cbdpstconf@sainc.com or faxed to (703) 797-4570.
